What we collect
When you submit a nomination we collect your first name, last name, email address, mobile phone number, the athlete's name, and the story you share about the athlete. We also record technical marketing information supplied by your browser: the page URL you landed on, the referring website, campaign parameters (utm_source, utm_medium, utm_campaign, utm_content, utm_term), advertising click identifiers (gclid, fbclid), a timestamp, and whether you are on a mobile, tablet, or desktop device.
Why we collect it
To administer the scholarship and contact the selected athlete's family, to send you the confirmation message, to respond if you contact us, and — if you gave consent — to send occasional training offers by email or SMS. Campaign data is used to understand which advertising produced a nomination.
Cookies and tracking
This page stores your campaign parameters in your browser's session storage so they travel with your entry. We may also load analytics and advertising tags (for example Google and Meta) that set cookies to measure page views and completed entries. You can block or clear these in your browser settings; the entry form still works.
Who we share it with
Our customer relationship management and messaging platform, our email and SMS providers, and our advertising and analytics platforms, strictly to run this promotion and our marketing. We do not sell your personal information.
SMS and email consent
Consent is opt-in and never pre-checked. Message and data rates may apply. Reply STOP to any text to unsubscribe, or HELP for help. Every marketing email includes an unsubscribe link. Unsubscribing does not remove the nomination from consideration.
Retention and your rights
We keep nomination records for as long as needed to run the scholarship and our marketing relationship. Email info@thomasfitnessandperformance.com or call 203-408-1108 to request a copy of your data or ask us to delete it.
Children
This nomination form is intended to be completed by a parent or guardian on behalf of a student-athlete. We do not knowingly collect information directly from children under 13 through this page. Youth athlete enrollment is handled directly with a parent or guardian at the gym.
